Alteration/Modification Forms
The submission process for Modification/Alteration applications has been moved online. The Modification/Alteration applications are now completed by submitting a Business Update application via the IDOA Licensing and Registration Portal. At submission, the appropriate fee is due for the application. Refer to the Fee Table.
