Media Request & Video Permit Form Posted on the IDOA-DCR Website

The Media Request and Video/Photography Permit Application is now posted and available for download on the Illinois Department of Agriculture – Division of Cannabis Regulation (IDOA-DCR) website.

This form must be submitted by the facility when requesting approval for media entry, video or film recording, photography, or similar activities inside a licensed cannabis facility.

As required under 8 IAC 1300.185, 1300.385, and 1300.485(d)(4), all media requests must be reviewed and approved by the Department prior to any onsite activity.
The form can be downloaded from the IDOA–DCR website under Forms & Resources.
